The Ketamine Institute has over 15 years of clinical and research experience in the application of ketamine based infusions for chronic pain an mood disorder in major academic institutions.    As a former Assistant Professor of Anesthesiology and Pain Medicine at the Yale University School of Medicine, I had the opportunity and privilege of working with members of the Department of Anesthesiology and Psychiatry in the clinical use and application of ketamine to treat chronic pain syndromes as well as related mood and addiction disorders. This included collaborating with members of John Krystal and Dave Charney’s group at Yale who published the first randomized, placebo controlled trial on the use of ketamine in patients with treatment resistant major depression

Additional work conducted while at Yale, Mount Sinai Medical Center and SUNY Stony Brook School of Medicine has shown that intravenous ketamine is a rapidly acting analgesic and anti-depressant that can achieve symptom reduction, improved mood and function in over 75% of patients who have treatment resistant depression or chronic pain within minutes to hours of administration.

Restore Patient Selection

An appropriate patient for the Restore Ketamine Infusion would generally be those suffering from a major depressive disorder or chronic intractable pain syndrome that has not responded to two or more fist and second line medications.


 

Mood Disorders - Depression / Anxiety / PTSD / OCD

Appropriate patients with a major depressive disorder without psychotic features would be those who have been treated with at least 2 different types of medication from two different drugs classes and has not obtained any significant benefit from therapy. In addition, patients who have failed to improve with bilateral ECT and rTMS therapy are also candidates for infusion therapy.

 

Patients with treatment resistant Anxiety Disorders, Obsessive Compulsive Disorders (OCD) and Post-Traumatic Stress Disorders (PTSD) may also benefit from Restore therapy.


 

Chronic Pain - CRPS/RSD / Fibromyalgia / PHN

Chronic pain patients who may benefit from Restore therapy include those with neuropathic pain syndromes that are not well controlled with opioids, injections, nerve blocks, neuromodulation therapy or or other prescription pain medication. Appropriate pain syndromes include complex regional pain syndrome (CRPS/RSD), trigeminal neuralgia, phantom limb pain, post-herpetic neuralgia, and migraine headache, etc. In addition, patients with chronic back and neck pain no longer responsive to any form of treatment or with other debilitating musculoskeletal and joint pain such as Fibromyalgia are candidates for infusion therapy.

Contraindications To Restore Therapy

Of course, there are some individuals with specific medical conditions who would not be appropriate candidates for this type of therapy.  In addition, several medications are known to compete at the NMDA/AMPA and/or the D2-3 receptor and therefore limited the clinical effectiveness of ketamine.


 

Pre-Exisiting Medical Conditions

• Known hypersensitivity (Allergy) to ketamine,
• A recent myocardial infarction: < 6 months ago,
• Unstable Angina / Accelerated Hypertension,
• Uncompensated CHF/pulmonary hypertension,
• Known / suspected Cerebral Aneurysm or AVM,
• A recent hemorrhagic stroke: < 1 year ago

Medication Interactions

• Risperdal (risperidone),
• Zyprexa (olanzapine),
• Lamictal (lamotrigine).

 

Any patient currently on these medications would require a downward titration leading to a temporary discontinuation for approximately 2 weeks before the infusion series and during the course of ketamine therapy. The medication may be resumed following completion of the infusion series if clinically indicated.
